WAIVER AND RELEASE OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT

I agree to IceLiners Synchronized Skating Team Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement as described herein. In consideration of participating in IceLiners Synchronized Skating Team activities, I represent that I understand the nature of figure skating activities (“activity”) and that I am qualified, in good health and in proper physical condition to participate in such “activity”. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the “activity”. I fully understand that this “activity” involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the “activity”, the conditions in which the “activity” takes place, or the negligence of the “releasees” named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the “activity”. I hereby release, discharge, and covenant not to sue the IceLiners Synchronized Skating Team, United States Figure Skating, its directors, officers, administrators, sponsors, volunteers, agents, employees, staff, instructors, trainers, other participants and if applicable, owners and lessors of premises on which the “activity” takes place (each considered one of the “Releasees” herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the “releasees” or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage, or cost which any may incur as the result of such claim. The IceLiners Synchronized Skating Team has the right, but not the obligation, to provide rules, regulations and/or ice monitors for Club Ice. We hereby acknowledge that IceLiners Synchronized Skating Team shall not be responsible for the supervision of the members at Club Ice. I have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect.
PARENTAL CONSENT AND INDEMNIFICATION AGREEMENT

I agree to IceLiners Synchronized Skating Team Parental Consent and Indemnification Agreement as described herein. I, the minor’s parent and/or legal guardian, understand the nature of the above referenced activities and the minor’s experience and capabilities and believe the minor to be qualified to participate in such “activity”. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses, or damages on the minor’s account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor’s behalf makes a claims against any of the above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss liability, damage, or cost any Releasees may incur as the result of any such claim.
CONSENT FOR MEDICAL ATTENTION OR TREATMENT

I give my Consent for Medical Attention or Treatment to IceLiners Synchronized Skating Team as described herein. This Consent for Medical Attention shall be binding and effective for the 2021-2022 membership year of the IceLiners Synchronized Skating Team. I certify that I, the member, or I, the parent/guardian of said participant, give my consent to the IceLiners Synchronized Skating Team and the facility the activities are taking place in and their staff and to members of the IceLiners Synchronized Skating Team, their Board of Directors and volunteers to obtain medical care from any licensed physician, hospital or clinic, including transportation and emergency medical services, for myself/ourselves and/or said participant for any injury that could arise from participation in these activities.
ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19

I agree to IceLiners Synchronized Skating Team Release of Liability relating to COVID-19 as described herein. The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Iceline Quad Rinks along with the IceLiners Synchronized Skating Team has put in place preventative measures to reduce the spread of COVID-19; however, IceLiners Synchronized Skating Team cannot guarantee that you or your child(ren) will not become infected with COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) may be exposed to or infected by COVID-19 by attending practices, competitions or events as a member of the IceLiners Synchronized Skating Team and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at practices, competitions or events may result from the actions, omissions, or negligence of myself and others, including, but not limited to, rink employees, coaches, volunteers, and program participants and their families. In addition, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. I understand that my child is not permitted to attend any practices, competitions or events if they are currently diagnosed with COVID-19, have recently been exposed to someone with COVID-19, has a fever above 100.4 degrees Fahrenheit, or are experiencing any other symptoms identified by the CDC. If I observe any unusual or significant hazard during my child’s presence or participation, I will remove my child from participation and bring such to the attention of the nearest coach/volunteer immediately. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that my child may experience or incur in connection with my child’s attendance at practices, competitions or events as a member of the IceLiners Synchronized Skating Team. On my behalf, and on behalf of my child, I hereby release, covenant not to sue, discharge, and hold harmless IceLiners Synchronized Skating Team, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of IceLiners Synchronized Skating Team, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in the IceLiners Synchronized Skating Team.
NAME AND LIKENESS RELEASE AND CONSENT AGREEMENT

I agree to IceLiners Synchronized Skating Team Name and Likeness Release and Consent Agreement as described herein. By registering for the 2021-2022 membership year of IceLiners Synchronized Skating Team, and for good and valuable consideration, I release and grant to IceLiners Synchronized Skating Team, Ice Line Figure Skating Club and U.S. Figure Skating, including its subsidiary and affiliated entities, and U.S. Figure Skating authorized local organizing committees, clubs, and interclub associations, the right to use my name, likeness, image, photograph, voice, video, athletic performance, biographical information and any other indicia of identity, in any format, medium, technology and platforms whatsoever, from any IceLiners Synchronized Skating Team, Ice Line Figure Skating Club or U.S. Figure Skating event or activity in which I participate during the 2021-2022 membership year and competition season (collectively, my "Identifications"), and to distribute, broadcast and exhibit my Identifications, without charge, restriction or liability, in any media now known or hereafter devised (including, but not limited to, television, internet, web casting, video streaming, social media, signage) or in various publications (including, but not limited to, brochures, flyers, marketing materials, SKATING Magazine, and the U.S. Figure Skating directory or media guide) into perpetuity, unless otherwise specified and agreed upon. I acknowledge that I will not receive any compensation, etc. for any such use of my Identifications, and hereby release the IceLiners Synchronized Skating Team, Ice Line Figure Skating Club and U.S. Figure Skating, including its subsidiary and affiliated entities, and U.S. Figure Skating authorized local organizing committees, clubs, and interclub associations, from any and all claims which arise out of or are in any way connected with such use. I HAVE READ THIS RELEASE AND CONSENT AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE.
ICELINERS SYNCHRONIZED SKATING TEAM SKATER CODE OF CONDUCT

Parents are asked to review the following IceLiners Synchronized Skating Team’s SKATER CODE OF CONDUCT with their skater.
I assert that I have reviewed the IceLiners Synchronized Skating Team's SKATER CODE OF CONDUCT with my skater and we understand IceLiner's policy regarding appropriater behavior by a skater. (1) I will give my best at all practices, classes, exhibitions and competitions. I will demonstrate excellent cooperation, conduct, teamwork and sportsmanship. (2) I will treat my team members and coaches with respect at all times. (3) I will focus my attention on the coaches during practice. (4) I will not engage in talking to other skaters during practice time. (5) I will not engage in freestyle skating during practice time. (6) If I have a question, I will ask the Coach and not another skater. (7) I will attend ALL practices. If a practice must be missed due to illness or injury, I will ask my parent/guardian to call the Line Manager. I understand that absences and tardiness may result in disciplinary action. (8) I will arrive 10 minutes before each practice. (9) I will wear my team practice uniform to each practice. This includes: team jacket, tank top, and Mondor pants and sneakers. My hair will be in a bun and up and out of my face. I understand that any outerwear may or may not be allowed during practice and is at the discretion of the coaches. (10) I will continue to practice my synchronized skating program during my freestyle ice practice and complete weekly video and skater skill log assignments. (11) At all competitions I will dress like my team. I will wear my team jacket, tank top, and Mondor pants and keep my hair up and makeup on until I leave the competition arena for the day. (12) I will not act in a manner that is disrespectful, illegal or immoral. (13) I will not be involved in any altercation that is an embarrassment to me or the team. (14) I will respect other people’s belongings and will not touch anything that does not belong to me unless I am given permission to do so. (15) I will not have a cell phone out in the Locker Room or rink side during Practice. If I bring my cell phone to practice, it must stay zipped inside my backpack until after practice concludes. (16) I will not use or possess tobacco of any kind, unauthorized drugs, vaping products, or alcoholic beverages of any kind. I will not attend events or parties where drugs or alcohol are being consumed or made available (family events excluded). (17) I understand that misconduct violations will result in disciplinary action. (18) I will follow the Coach’s request regarding additional skating necessary to improve my skills for the overall good of the team.
ICELINERS SYNCHRONIZED SKATING TEAM PARENT CODE OF CONDUCT

Codes of Conduct give everyone a guide to what is expected of us if we are part of an organization, participating in a sport, or as spectators at our child’s events.
I assert that I have reviewed the IceLiners Synchronized Skating Team's PARENT CODE OF CONDUCT and understand IceLiner's policy regarding appropriate behavior by a parent. (1) I will encourage good sportsmanship by demonstrating positive support for all skaters, coaches and officials at every practice session, competition and test session. (2) I will place the emotional and physical well-being of my child ahead of my personal desire to win. (3) I will encourage my child to skate in a safe and healthy environment. (4) I will inform my child’s coach of any physical disability or ailment that may affect the safety of my child or the safety of others. (5) I will teach my child that doing one’s best is more important than winning, so that my child will never feel defeated by the results of a competitive event. (6) I will never ridicule or yell at my child or other participant for making a mistake or blame my child’s teammates for placement in a competition. (7) I will do my best to make skating fun and will remember that my child participates in sports for his/her own enjoyment and satisfaction, not mine. (8) I will ask my child to treat other skaters, coaches, fans and officials with respect, regardless of race, creed, color, sexual orientation or ability. (9) I will applaud a good effort in both victory and defeat emphasizing the positive accomplishments and learning from the mistakes. (10) I will teach my child to resolve conflicts without resorting to hostility or violence. (11) I will be a positive role model for my child and other skaters. (12) I will demand a figure skating environment for my child that is free of drug or alcohol abuse and agree that I will not use or provide to a third party any drug prescribed by applicable federal, state or municipal law. (13) I will not assist or condone any competing athlete’s use of a drug banned by the International Olympic Committee, International Skating Union, United States Olympic Committee or U.S. Figure Skating, or, in the case of athletes, to use such drugs or refuse to submit to properly conducted drug tests administered by one of these organizations. (14) I will not provide alcohol to or condone the use of alcohol by minors, abuse alcohol in the presence of athlete members or at U.S. Figure Skating activities or, in the case of athletes, consume alcoholic beverages while a minor. (15) I will encourage my child’s coach to continue their education and training through programs offered by U.S. Figure Skating, the Professional Skaters Association and other accredited organizations. (16) I will respect my child’s coach and refrain from coaching my child or other skaters during competitions and practices because it may conflict with the coach’s plan or strategies. (17) I will respect the decisions of officials and their authority during competitions and test sessions and teach my child to do likewise. (18) I will show appreciation and recognize the importance of volunteers and club officials. (19) I will study the rules of the U.S. Figure Skating and teach my child accordingly, so that we have an understanding and appreciation of the rules of competition and membership.
